Commentary: Costs and Their Predictors in Transsphenoidal Pituitary Surgery

This paper1 explores the vast Truven MarketScan database (IBM, Armonk, New York) to compare costs associated with transsphenoidal microsurgical and transsphenoidal endoscopic approaches, using data derived from healthcare claims to multiple health plans and employers in 2010 to 2014. The authors report higher hospital/facility payments associated with endoscopic versus microscopic approaches. Payments to physicians did not differ significantly. Length of stay and complications were the greatest predictors of cost. These findings stand in contrast to some previously published studies which revealed similar or lower costs or cost-effectiveness for the endoscopic approach.
